HOW DO I BECOME ACCREDITED?


Initial Accreditation: If this is your first time applying for accreditation you need to submit the following:
  1. Application with Affidavit.
  2. Copy of a valid National Academy of Emergency Dispatch (EMD) Certification.
  3. Copy of current CPR, Professional Rescuer, or Health Care Provider card.
  4. Copy of a valid state or federal ID.
  5. Proof of affiliation with an approved EMD Center that serves San Joaquin County.
  6. Application fee (non-refundable).
  7. State of California Department of Justice Live Scan Background Check (must be completed no more than 90 days prior to submission of your Application).
Re-accreditation: If the applicant is being re-accredited, the following must be submitted:
  1. Application with Affidavit.
  2. Proof of four (4) hours of ride-along observation with an approved advanced life support provider.
  3. Copy of current National EMD Certification.
  4. Copy of current CPR, Professional Rescuer, or Health Care Provider card.
  5. Copy of a valid state or federal ID.
  6. Proof of affiliation with an approved EMD Center that serves San Joaquin County.
  7. Application fee (non-refundable).
Note: Only original, complete, legible applications will be accepted.

Application and Related Forms

* Live Scans processed more than 90 days in advance of submitting the EMT Certification Application will have to be repeated at the applicants cost.

Fees

All fees are non-refundable.

Application Fee: $33.00 (effective July 1, 2012)
Late Fee: $45.00 (effective July 1, 2012) for an application submitted after lapse
Replacement Card: $25.00 (effective July 1, 2012)

Personal checks are not accepted. Acceptable payment methods are cash, Debit, Visa, MasterCard, Discover, or money order. We strongly discourage sending cash by mail.

Q: How long does it take for my application to be processed?

It will take approximately 2 weeks for a completed application to be processed.

Q: Can I fax in my application?

No. You may bring your application in or mail it in, but it must have original signatures.
