Section 125 Plan


**The information below applies to the 2013-14 fiscal year.  Claims for the 2013-14 fiscal year will be filed with San Joaquin Health Administrators using the documents listed below. 




The Section 125 (Flexible Benefits) Plan is a voluntary program that allows employees to use pre-tax dollars to pay for health-related expenses that are not paid by a medical, dental or vision plan (Health Flexible Spending Account); and dependent care costs (Dependent Care Assistance Plan).

To save a form on your computer to fill out, RIGHT CLICK on the Section 125 Reimbursement Form link above and choose "SAVE TARGET AS". A box appears prompting you to pick a location and name for the file. Give the file a name and proceed to save the file on your desktop or a location of your choice.
